Our Complex Care Nurse Practitioner and Nurses provide ongoing nursing support and clinical oversight as needed. We work with you, your family, the hospital, your specialists, GP and Allied Health providers to coordinate your care and safely support your needs at home.
Our nurses and support workers are highly trained to care for those living with long-term conditions such as Parkinson’s disease, Multiple Sclerosis, Motor Neuron disorders, Spinal Cord or Brain injury.
Our Nursing team helps you with the following supports:
• Respiratory Management: Tracheostomy care, invasive ventilation, BiPAP/CPAP
• Enteral Feeds Management: Gastric Tube/PEG/RIG/J-Tube
• Medication Management: Inhaled, Oral, topical and Subcutaneous Injections
• Complex Bladder Management: Intermittent urinary catheterisation, IDC / SPC Care
• Complex Bowel Management: Colostomy care, Neurogenic bowel routines
• Wound Management
• Diabetes Management
• Seizure Management
• Palliative / Hospice / Comfort care at home
